intRODuCtiOn The self Adjusting File (sAF*) system is a new approach to shaping root canal systems, which does not use solid metal files. (Fig 1). The sAF relies on the gentle abrasive action of a flexible mesh lattice to prepare the root canal walls. The sAF offers: 1) the ability to instrument and prepare complex-shaped root canal anatomies; 2) the capability to provide continuous and efficient irrigation and debridement; 3) the potential for elimination of file breakage within the canals because there is no file rotation when cleaning and shaping and; 4) to simplify and reduce the number of required dental armamentarium. 9 Anatomic Considerations Root canals show great anatomical variation, and anatomical challenges include dilacerated roots, canal curvatures, fin-like projections, isthmus communications and internal resorptive defects. These complications prevent full instrumentation of all of the canal ramifications using currently rotary file techniques. Not all root canals are perfectly round in cross section, nor are they perfectly straight. Frequent anatomic irregularities mean endodontic preparation is a difficult and challenging procedure. Instrument-created distortion of the canal anatomy also poses limitations to traditional endodontic techniques. Classical endodontic instruments use a cutting action, which can gouge and deform dentin walls. This may create an artificially shaped geometry unlike the original canal shape with irregularities. procedural errors can include incomplete instrumentation and pulp removal, perforation, “zipping,” and thinning of canal walls. (Fig 2, 3).
